This is on the level of making the game completely unplayable from the constant flashes and noise from this thing. Seriously, you're going to give someone an epileptic seizure with this nonsense.
I found that for me it occurs after unlocking the 2nd and 3rd focus bar perks. Unfortunately I was too late to go back to an earlier save lest i lose a considerable amount of hours of gameplay. I however did find a fix. I found a mod which is basically a cheat for all intents and purposes that set infinite focus for only one bar of focus or also for second and third depending on your perks. The mod has other stuff too but you only need the v1 file for focus to get rid of the annoyance until it is officially fixed. However in order for it to work you need to enter combat at least once. If you load the game in a rest area such as Skyhold the bug will remain present until you travel somewhere and enter combat. then the focus bar remains at the first level in rest areas.
