When are you fixing Dorian's companion quest bug?

I've been having problems with Dorian, Solas, and Iron Bull not giving me their quests. Really not sure what I did, as I can't remember a time when any of them disapproved of my decisions. Visible approval rating would be nice to have, assuming that's really what's going on.
