We play DA:I together on Xbox One and are having the same issue here. My girlfriend and I usually play at same time on two xbox's, so no spoilers, but for whatever reason now one keeps freezing when we hit menu on the title screen. (lol which means I cannot play) Tried everything above, reseting xbox, re-installing game, etc., and even swaped game disks and DA:I continued working on mine one and not hers. This isn't the only glitch we've experienced too conversations getting stuck & an invisible merchant in skyhold just to give two examples.
Come on EA... I understand the pressure to put out a game on schedule but at least Ubisoft apologized for Assasin's Creed Unity's issues with one free DLC (or a free game for people who bought the season patch) and while glitchy at least the game was playable... I paid $60 and can't get pass the menu screen boss. How did these kidns of things get past your testers? I mean OK maybe the menu screen bug... maybe... I get it if it only happens on a small % of Xboxs because unless you load the game and test it on 1000 of them you might not find it but many of the other bugs/glitches are prevalent.
