Hey FabS7AR For your first question, as far as I am aware the Codes Entries won't update based on you gathering more information from the "More" option during dialogues.
In most cases...They give you additional lore/context on what you are talking about.
For your second question about how to get Codex entries, some will come from the main story, side quests, companions, but some can be found in the world. You should notice a pop-up on a few letting you know about them.
