I've yet to find any answer to this issue that seems to be cropping up for a lot of people. I've done a clean reinstall of the latest Nvidia driver, repaired the game file and uninstalled and then reinstalled the whole game and re-compiled the shaders.
Error can happen while playing, in a cutscene, in the menu, etc. Not straight away for me but after 1-2 hours. Normally.
Since it seems to happen to both AMD and Nvidia GPUs, it doesn't seem to be a driver issue.
