Assets, enemies, and textures not loading or activating

Frequently I'm entering into a new area, and various visual assets won't load in. Happens most often in the Solas memories in the Crossroads, some of the enemies and textures will load but not everything. This then locks me out of continuing the mission. Sometimes restarting the game works, sometimes it doesn't.
