Bug in lighthouse second portal puzzle doesnt appear

I've have a bug in the lighthouse. I cannot do the puzzle for which unlocks the second portal.  I have all the companions but there is no wisp  or holder where it should be showing up.  does anyone have a solution or has anyone had that problem?
