Bug issue: Game crashing

I'm playing on xbox series x and as soon as I start combat my sound will continue, my character will dissappear and it crashes my game and sends me to the dashboard almost everytime. After the second patch it seemed to be getting worked on because my character would dissappear but it would send me outside of the map instead of crashing but with the newest one it is back to the same. It's made the game unplayable because I can't get past a single fight without it crashing. I already tried all the steps to fix it (uninstalling, making sure xbox isn't over heating, clearing saved files and hard resetting my xbox) but nothing has helped
