I've been able to partially repro this issue by completing objectives for inactive quests. Tested this with the quest "The Warden's Vault" in Rivain Coast
The first attempt was without the quest being active from start to finish, and the unique chest couldn't be opened.
The second attempt was completing the quest half-inactive and half-active. The chest couldn't be opened.
The third attempt was with the quest active from start to finish as soon as the new quest prompt appeared. The chest was opened successfully.
THIS ISSUE IS A PROGRESSION BLOCKER for other quests, such as "Gate of Lost Ages" where you must interact with a statue to reveal a hidden path to a boss monster. This quest itself is necessary to complete another longer quest "Regrets of the Dread Wolf" which cannot be completed as a result.
