Yes this exactly - the most recent save seems to be the most reliable for recovering it, though on mine it does take a few tries - usually three. Once its there, I can progress in the game fine without my character appearance changing. But I can't go back to an old save without it happening, and once I've attempted to reload an old save the game won't load my character's custom appearance at all. So I then have to quit, restart the game and load the most recent save again in the hopes it will work. Very frustrating!

I think I figure out on mine it was continuing from an auto save caused it?
As soon as I dropped it and reloaded my last hard save I had my custom qunari face appearance back again.
