There are nor WER (Windows error reports in your DxDiag. That is a problem by itself but for now only for your information.
Is your chipset driver and your BIOS up to date?
I also would suggest to test with the newest studio instead of the game ready driver. Choose "Studio Driver" under "Download Typ"

I have checked the bios and chipset drivers. All drivers are updated. I tried clean studio driver but the result is the same...I got two screens would it matter?
