I've had a similar problem and what solves it is going to the Mirror and go through the three stages of the menu (on top of the screen, including gender, voice, etc.), without changing anything, then hitting "save changes" (even if you haven't made any).
(Haven't found a solution yet for "Baldy" - a bald, totally different version of my Rook showing up in the inventory menu from time to time, usually leaving after a change of scenery. I almost like him now.)
