Rook resets to default appearance every time I load the game

Every time I load the game up, my Rook's appearance is reset. In my first playthrough, I would have  to go to the mirror in the lighthouse every time to redo my entire appearance. But I've started a new game (after the patch update) and it is still doing it, and I haven't reached the lighthouse yet to redo my character so I'm now having to do the entire introduction as some dude I didn't create.
