Same problem here. Started during the fight with the lyrium ghoul, then i reloaded a previous save and it was fine. Except from that moment, everytime I exit the game and then open it again, no matter the save I load, Rook's appearence goes back to default.
Only thing that fixes is the mirror. With all the time spent in cc this is such a bummer ://
