[Spoiler] Missing Codex Entry despite completing requirements

Trying to unlock the achievement The Storm Quelled I have 2 out of 3 codex entries unlocked despite clearing all 3. Screenshots attached for the missing one

    Obtained save with the requirements done - It's now uploaded in nexusmods and can be found by searching for Human Warrior Warden Endgame Save and Secret Ending optional

Completed the game, I have all of the achievements... except for this one :/
