Ultimate Keeps Resetting to Default After Patch

I'm having this problem too. On at least one occasion it managed to reset to no ultimate entirely, which I didn't realize until I was mashing the 4 key in front of a boss and getting increasingly confused as to why nothing was happening.

Playing a warrior and have had this happen with both Heroic Leap and Warden's Fire equipped.
