So my daughter had this same / similar problem. I'm going to tell you the solution we found, but first let me say the fact that no one from EA or elsewhere posted this as a solution as it almost appears as if it should be a known issue is pathetic, but typical and expected from EA.
Anyways here it is. We had the problem on her computer with a Sims 4 update just being stuck on "Preparing" forever. As in hours. No progress no error, it was just stuck there. We tried reboots, we tried clearing cache, etc... nothing worked.
FInally I poked around in the app settings and found a check box / slider bar to enable automatic downloads and downloads in background. Immediately after enabling both it went from preparing to downloading and the update installed successfully.
Why would you need this on? Doesn't make sense. Why would manually selecting to update with these settings off cause it to fail? No idea, and it doesn't make sense. But crappy software works in crappy ways.
I don't know if this will work for others, but it worked for us despite making no sense and after hours of frustration and annoyance.

I haven't played sims 4 in a long time but am tempted by their current sales. Again a million thanks for fixing it when official help doesn't. As soon as I enabled automatic downloads and downloads in backround a PROGRESS BAR showed up and it's now updating sims 4.

- @916b440868cdef5e Thanks! That seems to have done the trick! Mine was stuck on "preparing" for 3 hours. I was just about ready to get my old Sims 3 discs out of the basement and install them the old fashioned way. Maybe now I don't have to.

Hey All,
There have been some questions on this one, so here is where to locate these options to get the content download to kick in. Thank you to @916b440868cdef5e for the steps on this one.
- Launch the EA app
- Click on Settings in the upper left corner
- Click on Applications
Make sure you have the following enabled
- Automatic downloads
- Downloads in background
I will also go a step further and clear the cache from the system.
EA App's Cache (Top Left Menu > Help > App Recovery)
Deleted my previous comment because it was unusually mean-spirited of me and, somehow, I got it working. I had to stop all my downloads first though, which may be the crucial part for some folks!
But please, EA, for the love of all that is holy, in the name of Jesus, Buddha, and Spode, FIX YOUR APP.

What happens when the bug occurs? The game will not download from the EA app.
What do you expect to see? I want to see the game launching and the app loading.

I still can't get Sims 4 to get past preparing. I have been trying for 3 days now. Everything that was said to be a fix and I still can't get it.

You are genuinely a life saver? I did eveything EA had put up, was going through reddit and nothing and then came across here and saw you solution and boom- worked straight away. Sad to see that even after a month of you posting this, EA haven't decided to write this solution or even attempt to fix their [removed] app. None the less, thank you so much <3
