It sounds like you're aware of the linking process itself, but the full details can be found on this page. As you mentioned -
You can only unlink and relink your EA Account to a different Steam account if the previous account link is more than 12 months old. If it’s been less than a year, you won’t be able to relink yet.
If you're past that point and still having issues with the link, please contact support directly so they can take a look at the account, as we're unable to make any changes here on the forums.
