If repairing the game via right clicking in Origin and also resetting the GPU default settings via the nvidia control panel do not prevent this visual issue, please post your Dxdiag report here so we can take a look, thanks.

Nothing really wrong in your Dxdiag.
Make sure you have all available Windows updates installed.
Can you check if the same issue occurs when you play on one of your other monitors?
Maybe disconnect your Occulus and check on the issue again.

I personally dont think that the oculus could be the issues as i had mine connected aswell and didnt have any issues, but yeah, this seems more like an issue with the nvidia controller panel. Try to reset it to defaul settings and than disable VSync, thats how i could fix a similar issue I had in BF1
