PlayStation Trophies Not Being Granted
I have seen a few threads around on PS trophies not being awarded to players after meeting all the requirements. The two I have seen the most are the following but there are more being mentioned else...

did you complete it in a solo or multiplayer game session? Multiplayer for Helpful Heart, solo for Bewitched by Bathing

Helpful Heart did not pop even after successfully reviving a Hunter twice.
All 6 baths unlocked and used, and no trophy pop.
