I need help to find my created player in career mode

I played 2 seasons with my created player and loading into the new 3rd season he disappeared from my squad and is replaced with a random ai player with lower stats(61ovr). In the first season he was 85 rated and before I lost him he was 95 rated, everytime I press the squad hub it says an error happened and will always log me out of the game. DOES ANYBODY KNOW WHY THIS HAPPENED AND HOW I CAN GET MY PLAYER BACK PLEASE!
