PC - best graphical settings?

I have a fairly high-end PC and I'm looking for tips on the best/most responsive graphical settings. Vsync on/off, FPS limited or not, full screen or borderless window, how about refresh rate? And most importantly, what is the best combo between these? Thanks in advance!
