Unfortunately I do not have another PS+ account to log in to UT with…
EA_Aljois there anything you can do from your side on the back end to refresh my game’s or account’s cookies/permissions?

No. There isn't. We don't have cookies on our end. If your account was banned from UT, you would have received an email saying so.
Try power cycling your modem/router by unplugging them for 2 minutes. Also, hard restart your PS4 by holding down the power button until it turns off. Then, turn it all back on and try again.

EA_Aljothank you for the comprehensive response. Unfortunately none of those troubleshooting steps seem to work. I put a lot of hours into this UT and would be very disappointed to lose my god squad forever.

Try restoring the licenses with the steps here. Also, rebuild your database and clear the system cache with these instructions.
Your team isn't deleted. You just can't access FC UT. Hopefully, what I suggested will help.
