"Three Players 95+ Futties pack" with a gold rare player

I recently opened a 3 Futties pack but I only got 2, did anyone ever have this issue? How do I get the one I'm missing? I sent a few e-mails to EA Help but they didn't help at all.

Also, is it possible to get 98 and 99 overall players from the 3-Futties players pack? I've been playing for months and I only ever got 95 and 96 with few 97 overalls, but never 98 or 99. Not to mention I get lots of duplicates. Just today I opened a pack and all 3 Futties were duplicates.
