Missing FC Club Member Rewards

Hello, 

I am still missing the FC Club Member Rewards, such as the kits/badges/etc, that was shown to me when starting the game (see image attached) and described here.  I've followed the guide for missing rewards, however I still haven't received the items after a weeks time.  Please let me know if there is anything needed from me to help resolve this issue!  Thanks
