Formation keeps getting changed automatically midgame without using quick tactic

I have my team setup with a specific formation. Mid-game the formation gets automatically changed without me changing it in Team Management or using the quick tactics.  Randomly getting my formation set to 4-5-1 or 4-2-3-1 and its throwing my team out of sync mid game by moving the players into the wrong positions.

Why is this happening?  I have only one tactic setup as I was suspecting it may be related to having multiple formations saved - this did not help, it still keeps auto changing.   This was a formation imported by a code, and it always reflects correctly and the game starts correctly with the right formation.

I am on PC.
