screen tearing and fps spikes

I bought the ultimate edition .

I have a RTX 3060 6 gb GPU and I have an insane screen tearing although I have the settings on low and off and capping the refresh rate and frame rate limit to 60 fps , and with turning on the v-sync , the screen tearing disappears but feels like the fps is dropping but at the same time my fps counter reads 60 (nvidia overlay) and doesn't change . 

what should I do ? I tried literally every thing to have a smooth gameplay and it is not working , AND PLEASE LET ME KNOW IF THAT WAS A BUG .
