Lag Comepnsation

I play on PC and regularly match with console players. Some matches are smooth, while others have severe input delay, poor hit registration, delayed damage, and strange movement. 

Is this caused by crossplay, lag compensation, server performance, frame-rate differences, or netcode desync? Are there known issues, and what can be done to fix them? Please provide a clear answer.
