Typically, this refers to a login per day. Always make sure to start a new session after the daily reset. Check tomorrow to see if it counts as your 4th login.
Regarding Ramadan, I explained this in another thread earlier this week, so feel free to search the forum for a detailed answer.
Short version: For the 2nd part of the Ramadan Challenge, there’s only one challenge, but it gives you 5 points because it includes 5 games. There will be 3 more multi-day Ramadan challenges, which will bring the total to 20. Each time you complete one of these challenges, you'll earn 5+ points.

Regarding Holi: I exit and restart the app everyday to make sure my daily quests reset (as we all know they sometimes don’t do so on their own), so it really does appear to be a bug that I’ve only been credited with 3 Holi log ins in over a week of play.
