Is create a case gone?

I can't seem to find the create a case. It's been there these past few weeks that I've been playing this garbage again this year. I am trying to make yet another case for being lagged out of a game of Madden. 38 seconds left in the 4th, I'm up 24-6 and out of nowhere the game just locks up. I have lost count of how many cases I've submitted this year alone for Madden. They do nothing about them but I still want to report this guy.
