Running Back Motions on a Running Play and No Slot Receiver

Summarize your bug When I select Singleback formation, my running back immediately motions to the slot. Also for any formation with 3 wide receivers, it places a random(?) player as the 3rd wide receiver (could be a linebacker, defensive end, etc.). I have 5 wide receivers dressed for every game. Sometimes there is no one at all as the 3rd wide receiver, and I only have 4 eligible receivers. These issues started shortly after my usually slot receiver got injured for 5 weeks; I don't know if this has anything to do with it.
How often does the bug occur? Occasionally (10% - 49%)
Steps: How can we find the bug ourselves? I have no idea.
What happens when the bug occurs? When the running back motions out, he is simultaneously the slot receiver and running back motioning...
What do you expect to see? The running back to stay in the backfield at the start of the play. And, a 3rd WIDE RECEIVER put in instead of some random player.

As you can see in the video, the receiver icon is there but not a receiver. Also, the number of the slot receiver is 0 when I go to pick the formation.
I call a running play, and the running back motions to the slot for some reason. Again, there is no slot receiver.
I uninstalled and reinstalled already. I recorded these videos after the reinstall.
