I have the same issue but with android, madden15 was the only game where I was able to buy cash, last season the glitch appeared and since then I cant buy any cash at all.
I did all the steps you mentioned and still running without the chance to buy cash.
Hope some solution can be found.

Tried the uninstall MM, log out of iTunes, etc. fix and it didn’t help. You all don’t respond to this issue when I email you. I have been a good customer and have spent $100’s of dollars with you. Now I have invested all this $ and can’t do anything else to improve my team. Please help fix this!

I tried all of those steps three times.
i was on the phone with EA twice today
the first time they blamed Apple.
i called Apple They found nothing wrong
I can buy items on the Apple store and every other web site or app.
i called EA again. they called back had me on the line for 47 minutes before they disconnected me.
no one called me back.
Tthis is horrible.
there are good packages to buy now and they won’t take my money.
id give up except this year alone!
