Decryption Problems

There are several people having a problem with the decryption mini game in ME1.  The problem is that the mini game is auto failing regardless of whether the correct button is pressed withing the allotted time or not.

I have played ME1 since I bought my first xbox 360 (elite) back in 2008 and have played over 20 characters, so I can say I know how the decryption mini game is supposed to be played.  I upgraded to the slim 360 when the MW3 edition came out and had no issues with the game on that console until this last August.

That's when I downloaded the Leviathan update for ME3 (and also received the console updates and other game updates at that time).  Since August I've had this issue.  While using one of the MW3 controllers the mini game would auto fail on the first button pressed.  When I switched to one of my older controllers it was the third button pressed in the sequence.  Given that my video was starting to get glitchy while playing ME3 multiplayer I thought the console was going bad.  So I recently upgraged to a Halo 4 edition.  I also redownloaded all my DLCs rather than transfer the hard drives/transfer the data from the hard drives.  However, even on the new console, with the Halo 4 controllers, the mini game is still failing.  This time on the 2nd button in the sequence.  Note, that this is only for lockers.  The computer in the embassy was "hacked" as normal.

I have read about this issue on the bioware forums, and this issue seems to be on the xbox as well as the PS3.  I have not found any response on those forums that gives a solution to this issue.

These threads were added so that anyone that wishes to check out the bioware forums to see if I missed the "answer" may do so, or to use it to research an answer.  The former thread gives a solution, but that solution isn't to the problem I described.  Any assistance on this matter will be greatly appreciated.

I have been having some trouble with the bypassing feature on my copy of Mass Effect for the Xbox 360 (250 GB slim)

I for whatever reason cannot pass a single bypass (can't even get past the first button) every time I tried to play over the past 2 years, I know this may sound like a case of "work on your QT reflexes", but I have not had this problem before I got my 360 slim (I don't know if that may be the case).

The speed of bypassing on lockers and computers is obscenely fast, I hit the indicated button as soon as it flashes but no matter how many times I try (there are times I spent 2 hours tying to bypass, failing, and restarting a save) it will not pass.  I won't lie, I do not think I am that horrible at QTEs, but even if I were I think I could score at lease 1 successful one.

I know this may not seem like a big deal, but I noticed this on ALL Shepards I create.....and when it comes to the Mass Effect universe I tend to be a perfectionist, in both story and game surroundings.

Is there ANYTHING I can do?  I am hesitant to play because I know that having to constantly use omni-gel to open devices is going to nibble at the back of my brain (in the past all my Shepards could hack and bypass flawlessly......now they have to use the common man's Omni-Gel lol)

please help, please help me leave no locker unopened, no computer unhacked.......please let my future Shepards live in a world where they can bypass as well as my Shepard prime.....before (what I am thinking may be) my 360 slim supercharged the bypass' QTE speed.  Or whatever caused my QTE bypass speeds to be turned up to 11

I have looked at the boards and looked at the mod's reccomendations, and I am doing everything right (boosting my talents in decryption, I bring Tali with me with maxed out skills, etc...)
BUT as I said, I played this game religiously and I know for a fact that the QTEs were not this unhumanly fast
