Along with this new "feature" where I can't return to full screen mode without using Left Alt+Enter, I noticed that when I return, the numbers show next to the dialogue options.
Anyone know how to remove them again?
Yup. Reset your bindings to default, then scroll down to the bottom of the bindings list and clear the bindings you don't want.
You have to actually reset your bindings to default, because if you just clear, they don't get saved in your preferences when you exit the game.

Thanks for your answer but it turns out that I just didn't understand what was happening. Whenever I had to alt-tab back in before, I didn't have to Lalt-Enter before so when I went back in the game, my controller was the last thing the game new so nothing changed.
Now, once in the game, I Lalt-Enter and the game switches to mouse and keyboard mode so this is resolved automatically as soon as I use my controller again.
I did try to use what you stated to see if I could switch things back to only needing alt-tab to go fullscreen but I couldn't find a way.
