Okay, strange. I reloaded to a save from two days prior, repeated several missions, then finally got back to the vault and tried it again. This time it worked. Weird. I wonder why it didn't work the first time.
I think it might have been that you didn't load an early enough save which happened to me on my first playthrough. You must make sure its BEFORE you enter that area because if you touch any other console and then try the last two it won't work. why they haven't implemented a reset function i don't know. but it it is always those last two consoles, five then four. So i am pretty sure this particular puzzle isn't random but some are elsewhere.
Can never have too many save games. i kept about 10 in rotation for ME:A.. always saving over the oldest so that i have 9 fall back saves for points in time.
I do the same have 8-10 manual saves that I rotate and then of course there are also the autosaves.
I tend to manually save before big fights just in case it goes seriously wrong. It hardly ever does but better safe than sorry.
