Broke for me too which is hugely annoying after having spent many painful hours doing ME2 insanity.
I romanced Liara in LOTSB and didn’t get Paramour II, is that intended?
@TaylorPYV Unfortunately it is intended and doesn’t count as it never awarded the paramour achievement in the original version of ME2. You could two time with someone else to get the achievement if that’s still possible at your stage of the game otherwise you’ll be out of luck.
Quick side note: Paramour II doesn’t seem to be working properly in general at the moment. I’m in the endgame and hoping for a quick fix to all these achievement bugs so I can move on to ME3

I restarted with the intention of completing insanity achievements without changing career (which I have an inkling is the problem).
Re-played ME1 on insanity (which popped 'Insanity II' on Xbox). Incidentally, re-playing the romance scene popped 'Paramour II'.
Yet the achievement log in my cabin and the Xbox achievements list are out of sync, but I'm less concerned by that - that should fix by patching.
