Just completed my Insanity run of ME2 about 30 minutes ago, no achievement to be seen. Playing on PC, this is the first one that hasn't popped when appropriate. Super bummed, insanity is no joke. I have a friend playing on PC, same as me, and they got the achievement. Our games were practically the same: imported ME1 character but swapped class, completed all loyalty missions and miscellaneous assignments. I'm thinking it must have been the most recent patch that did this, since they finished their run before the patch hit.
EDIT: I am playing on the latest patch (from 6/7/2021). I imported my character from ME1, where I also completed an insanity run, so in theory the difficulty has never changed, not even across games. Just reloaded my old save from right before the Reaper IFF mission to play through Arrival, then re-do the Reaper IFF, Legion loyalty mission, and the final mission. Achievement still did not pop. Guess I'll be doing another Insanity run once this bug is fixed. :/
Just completed ME 2 Insanity run near an hour ago, all achievements have been unlocked without any issues (PC, Steam). All content was completed before the omega 4 relay jump.
I am still having the issue one Xbox one X. Completed the game, but didn't get either Paramour II or insanity II. I am on the latest patch.
This is a little frustrating. Good thing I have a save before the omega replay in case they decide to fix this issue.
