It could be because of a broken savefile. I would try first try and older save file to see if the mission is working in that. Secondly, scanning the harddrive for errors is always a good idea, all mechanical objects can make errors which in this case could result in a single file being written wrong: http://help.ea.com/en/article/how-do-i-run-chkdsk/
Finally got the chance to play some after checking the disk...still doesn't work.
Did make the N7 helmet at the Normandy crash show up, tho.
