The Game is bound to the account you registered it to. Are you sure that you use the same account you used to register the game?
If so, you should contact EA directly. If you are not from the US change the country in the top right corner.
Well, I've found two different accounts that relate to Bioware, Electronic Arts, and Origin. Neither will accept the code in the box. I finally managed to get a ticket submitted and had a fruitless wait on hold for chat assistance. Is there any other company or site name that I might have registered this game under when it was new?

No account, not even the one you used to register the games in the first place will accept the code for a second time. In the moment you entered that code for the first time, the Game was bound to the account you used at that time.
If ME3 is not showing up on either account under "My Games" in Origin (make sure that under "My Games" at "Show" > "All Games" is selected, also look for "Hidden Games" under "Show") the game is registered to another account.

No, the game must be registered to a EA Account and Origin was logged on with the credentials of that account.
If you have problems wit the chat, change the browser you're using, especially if it is Chrome. If that not helps, restart your router.

The link https://help.ea.com/contact-us was especially helpful once I figured out that what I needed to select was 'Call Me Now.' After one false start, I got an articulate, informed advisor who was able to combine my two accounts and walk me through getting my Origin client correctly connected. So now everything is fine!
