Yep, getting this one, too. Triggered it by talking to Lexi after collecting Jaal from Aya on my New Game Plus character, on PC.
Starting to feel like this game is far too unstable to keep playing. I'm going to try and make a new character, but I was really looking forward to tackling new game plus. This is very frustrating.
