I've got all three insanity achievements by playing the final mission in Mass Effect 1 on insanity three times.
The paramour achievements still don't work even romancing three times in one game. I have romanced Liara in both ME1 and ME2 and only got Paramour 1.
If I look at achievements in Mass Effect 1 then Paramour 1 shows as unlocked, if I jump to Mass Effect 2 and look at achievements Paramour 1 is blacked out, when I jump back to ME1 to double check Paramour 1, it becomes blacked out even though it was unlocked.
Yes, it seems the in-game achievements aren't synchronized, like I tried to explain in my case :
Really hope this will be fixed in a future update, because I don't see how to get Paramour II or Insanity II achievements in actual state ☹️

I've just had an update for Mass Effect Legendary Edition on my Xbox, it's only about 300mb big but after the update I checked the achievements in game and all three insanity achievements and paramour 1 was showing as unlocked, so hopefully this update has fixed it but it seems Liara in ME2 doesn't count towards Paramour 2.

Still excluding Liara from the achievement in the "legendary" edition is so fkin cheap. The only way this achievement string could have been done in a single playthrough while staying loyal.

Yep, looks like this not only fixes the achievements not triggering but credits those already achieved.
