Operation Lodestar (Insanity) Commendation Pack, Completion Acknowledgement Under N7HQ's Past Weekend Challenges
I completed Operation Lodestar on Normal & Insanity at the same time yet received only one commendation pack. Also, now my N7HQ shows Lodestar (Insanity) as greyed-out under Past Weekend Challenges, although if you scroll up under the Challenges tab it's clear that I completed the task.
