Is Wrex with you on that mission?
Did Mordin survive ME2?
Saving the cure alone isn't the only variable.I think she is supposed to die. I have just finished this quest. I think the point of the quest is Shepard's decision making about the espionage and his action towards Mordin in the end.

I agree with Keeper on this. Eve has survided in my games because:
1. I made sure to complete Mordin's Loyalty quest in ME2.
2. Mordin survived the Suicide Mission in ME2.
3. Wrex survived in ME1 cutscene where you had the choice to blow him away or convinced him you are right in destroying Saren's genophage cure.
Yea Wrex is alive, mordin is alive, mordins cure was saved, the bomb was disabled ect ect
I have done everything in the game that i need too mechanically but for some reason Eve ALWAYS dies. She is not meant to die, i have seen youtube vids of her alive and well after curing the genophage.
I can't understand why this is happening.
And i have done several playthroughs, each one insuring what i need to do is done, and then double checking and AGAIN she will die. I do not understand where i am going wrong. I have been making specifically sure that i do everything right but it makes no difference.

There is only one variable that matters regarding Eve’s surviving in Priority: Tuchanka, and that is that Melons cure in ME2’s “Old Blood” is saved.
So imho the only explanation in your case is that the imported ME2 save game has the wrong flag set.
You can check this by exporting the Save Game that you imported to ME3 to a PC and check the flag with gibbed ME2 save editor.
Edit: You also can export an ME3 Save and use gibbed for ME3 to check or alter the variable.

Just to chime in to help: I have the same issue (Eve dying during mission). I have reviewed the save flag from my ME3 save game, AND the save game that I imported from ME2 and they are right (Saved Maelon's data, Maelon lived, Wrex is alive and is a friend, told Eve about Salarian sabotage, Talked at length with both Eve and Mordin in the ship prior to the mission, etc.) EVE still died. I have played through ME3 20+ times and this is a new issue for me. The only thing I can think of is that I have been editing the coalesced.bin files and that might be corrupting the save game file somehow (I have seen erratic behavior prior after edits in the past once or twice).

Solved it (And a big thank you to those on the board who mentioned using the Gibbed.MassEffect3.SaveEdit tool)!
- For my fix of this issue, and using the program "Gibbed.MassEffect3.SaveEdit", I could see when I opened my save game files that I had saved Maelon's data, yet Eve was still dying in my ME3 save regardless (She was even coughing in some dialogue). I opened a save game right before I started the Priority:Tuchanka mission, then I saved a new save game. In this newly saved game file, I first edited the save game using "Gibbed.MassEffect3.SaveEdit" to UNSELECT saving Maelon's data, I then overwrite this save, then I re-open the save file using Gibbed.MassEffect3.SaveEdit toll again, and toggle the save Maelon's data back on again, and finally overwrite to the same save file one last time.
This allowed me to talk with Mordin about the Saved data and also Eve was safe at the end of the mission.
This is definitely a bug of some kind, but I cannot fathom if it might be from a corrupt import from ME1 to ME2, or ME2 to ME3. It also might be the massive amounts of Coalesced edits I do for ME1,ME2,ME3 also as I said before.
I have done everything right. Wrex survived me 1, saved Maelon's data, disarmed bomb, revealed Dalatrass sabotage, and Eve ALWAYS dies. Only happens in legendary on Series X. I've completely deleted all saves, uninstalled and reinstalled game, checked.every box along the way, and Eve always dies. Please no smart * comments saying I must have missed something, I didn't. I reinstalled the game last December after finishing it multiple times for different endings, and since I've reinstalled it I can't save her. IMO, the game has a now permanent flaw in code. Only other possibility is there is something required in ME1 besides saving Wrex. This had been my favorite game series. It's now unplayable.
