After your first meeting with the council, Ashley and Kaiden have a small amount of elevator dialogue that seems to be lower of volume compared to any other instance in the game.
It could also just be the character combination, but regardless, their volume is lower than other elevator dialogue instances in the citadel.
I also have this problem, but it has happened in all my elevator scenes so far at the Citadel. I've only just become a Spectre, but the problems has occurred consistently and squad-mates are barely audible.
News reports seem to be fine. Could be Ashley as she has been a common factor for me
Don't think I had this specific issue. I'm pretty sure the sound is out of sync though. Some parts of random dialogue would drop in volume.
