Recommend reporting bug inside the game on Bioware Social Network, that way the developers can see what you've posted more easily.

I'm happy to say it's still not fixed in 2020 just happened to me I had to just keep holding back when he got "off" the ladder and fell off the building.

@ColeTCG Change your Game Graphics Resolution to 800 x 600 in Windowed Mode and it works 1st time Every Time.
