Re: Problems with ME3 saves.

a) What platform do you use?

b)  Did you load a "Named Save", a "Quick Save", or an "Auto Save"?  I've noticed that if I'm not careful I can load an earlier save than the one I expected/intended because the first three are listed in a fixed order, not in the order they were made. (Just happened to me last night.  Only had to re-play about 10 mins, but I facepalmed when I realized what I'd done!)
