I mean with games like this one, I am sure it's probably hard to track everything you expect a couple of times this occurs but it does seem like a lot in Andromeda.
On Kadara I saved Sloane. While leaving the quest area, Sara mentioned something about the outcasts. I fast traveled back to the slums and on exiting the nomad got the exact same line only menti9ning the collective protecting the new outpost instead... like umm no. I kept Sloane alive, the outcasts are protecting not the collective.
Sadly so many little things can break immersion. As much as Imlove this game and the characters (I really do ) so much feels unpolished, incomplete and rushed. This game has so much awesome, its sad to see other things ruining it.
Yeah. The freedom of doing quests at your own leisure leads to these intertwined conditions. They must be very thorough with their check lists, checking loose ends and conditions to minimize these problems. They mentioned somewhere they would be fixing some 'off timings' here and there, but it looks like there's quite a lot of work to do.

Yup and ironically SAMs a little * after the patch(I dont remember this happening before the patch). Typically after I finish a mission I will check my emails. Then go talk to the crew. After making my rounds and returning to the Bridge SAM will announce that "there is new email pathfinder." When I check, nope... no emails. already read.... and the damn AI in my head should know that!!!!!!

This is the Yeoman's Syndrome, don't you know? Remember Kelly Chambers back in ME2? After we read our e-mails, she would often say, "Commander, you have an unread e-mail in your private terminal."

Yeah, which means the script that fires that bark doesn't have an associated sanity check. Check to see if the messages that should be there are there and unread before issuing the notification.
