These bugs haven't and won't be fixed because Mass Effect 3 is no longer being supported by the BioWare Edmonton developers. They have long since made the transition to Dragon Age Inquisition (which has now been released) and a yet unannounced game series. I believe support ended a year after the game's launch.
The ladder glitch is not really a glitch. It's a synchronization issue caused by a bad connection with the game's host.
