Had this happen to me as well. Earthborn War Hero, had not completed Hades' Dogs yet for reference. Toombs' and Shep's dialogue made sense until I chose the paragon option, at which point Shep says something about "I couldn't help then, let me help now" like in the Sole Survivor background.
IIRC this is an issue from the original game.
