Why didn't I get the achievement for finishing ME2?

Okay, to start this post, let me clarify that I know EA app's achievements are screwed right now. I'm already running the firewall workaround, and I got the achievement for going through the Omega-4 relay. But I'm missing the one for finishing it, finishing it with everyone alive, and for finishing two games on insanity. Any idea why these didn't pop???

Hey, so, it's been a month, and I'm still missing the following achievements:

Mission Accomplished (ME2)
Against All Odds (ME2)
No One Left Behind (ME2)
Patriot (ME3)
Legend (ME3)
Last Resort (ME3)
The One and Only (ME3)
Insanity II
Insanity III
Long Service Medal

I have finished ME2 twice on insanity mode, and I'm almost done with my second playthrough of the trilogy with the same character. I am still using the firewall trick which got other achievements to pop but these ones seem insistent on remaining locked no matter how many times I finish each game. Any help?
